LMKI Forms

These forms are in Adobe Acrobat format. You must use the Adobe Acrobat Reader to download and view most of these forms. They have been approved by the Corporate board for all leagues and should be included with all season and All Star registration information packets. These individual forms are also included in the annual package and the current version of the Official League and District Rules and Regulation and Corporation Bylaws . Note - files labeled "can edit pdf" should be downloaded and saved to your computer before you enter information.

Scholarship Application (can edit pdf) (this form may be used for Little Miss Kickball and/or local league scholarship requests) LMKI applications must be postmarked by April 15th. LMKI offers scholarships to graduating players for continuing education expenses.

Rule Books are typically available for sale in league concession stands. The current book covers the 2008-2011 seasons. Their sale helps our Scholarship fund.
